What do you need for your appointment

Find out what you need to bring for your first or review appointment and associated consultation fees.

Dr Pullen’s approach to your care:

My aim is to ensure you fully understand your diagnosis and the different treatment options available for your unique situation.

Please feel free to ask any questions and bring a family member along for support.

First appointment

Please bring the following to your first appointment with Dr Pullen:

A referral letter – A referral letter from your GP or specialist is required to see Dr Pullen to ensure that you are paid your Medicare rebate entitlement. GP referrals are usually valid for 12 months from the date of attendance and specialist referrals are valid for 3 months.

Imaging Requirements – Please bring all relevant X-rays and scans with you to your appointment. Dr Pullen may send you for further scans or investigations if required.

Workcover and TAC Patients – Please bring a Workcover Acceptance of Claim letter or TAC Approval for Consultation letter to your appointment.

If you do not have this at the time of your consultation, you will be required to pay for your visit. You will be issued with a receipt, which you can submit to your case manager for reimbursement.

Prior to your appointment:

  • Please complete Patient Registration Forms and return prior.
  • Please complete list of Medications and known Allergies.
  • Workcover or TAC patients – Acceptance of claim/consultation letter

Please bring to your appointment:

  • Medicare card
  • Private health insurance details
  • CD or films
  • Current referral
  • List of medications
  • List of known allergies.

Review appointment

Please bring the following to your review appointment with Dr Pullen:

A referral letter – Our staff will inform you if you need a new referral from your GP or specialist.

Imaging Requirements – Please bring all relevant X-rays and scans with you to your appointment.

You may be asked to update your:

  • Patient registration information – To ensure we have your up to date details
  • Medical information – List of medications and known allergies as these may have changed since your last visit.

Please bring to your appointment:

  • Medicare card
  • Private health insurance details
  • CD or films.

Please ensure you have a valid referral from your GP or specialist prior to your scheduled appointment to ensure you are eligible for a Medicare Rebate.

Accounts

Please pay your account at the conclusion of your appointment. You can conveniently pay by:

  • Cash
  • Eftpos
  • Visa
  • Mastercard

Please note – We no longer accept personal cheques for payment of consultations.
